The Guide didn't respawn.

You have to make a an 8 by 14 house with a good amount of lighting, doors, and one table and chair in it. Then he guide will respawn and live inside of the house, I assume this happend because you didn't do the listed above.

Yes, it should respawn if you do it exactly right. And be patient because he may not spawn immediately.

The guide will only respawn during the day, never at night. A respawning NPC follows the same rules of how he can spawn as a newly spawning NPC like the Nurse, the Dryad, the Merchant, the Arms Dealer, etc.
