Divide the decimal number by 16. Treat the division as an integer division.

Write down the remainder (in hexadecimal, ie. if the remainder is 12, write down "C").
3
Divide the result again by 16. Treat the division as an integer division.
4
Repeat step 2 and 3 until result is less than 1.
5
The hexadecimal value is the digit sequence of the remainders from the last to first. (So if you had 001, it's 100.)

And while you may not care about this, I think kando might be.
I figured out an equation to convert HEX to DEC :D (I feel so smart today)

Let's use AB as our HEX value.
To convert it to DEC, all we have to do is set the first bit "A" as "X," and the second bit "B" as "Y."
So our equation looks like this: 16X + Y= our DEC value
So let's plug our byte "AB" into the equation.
16(A)+B=
In HEX, A=10 and B=11, so
16(10)+11=
160+11=
171

And there you go. (Kando, you posted before I could submit)

also, if its 3 digits, like 123, its (256*1)+(16*2)+(1*3)

to reverse, like 171, you have to divide...

Let's convert 256 to hexadecimal. Divide it by 16: 256/16 = 16.
Since it has no remainder, put "0" as a remainder.
Divide 16 by 16, which is 1. No remainder, so again put "0" as a remainder.
Lastly, since we know that 1/16 is less than one, just put 1 as the remainder. Our remainder list is 001, so we switch it around and get 100. 100 is hexadecimal for 256!

2500

This gets more tricky. Divide 2500 by 16, we get 156.25. Multiply our remainder, (.25), by 16. This gives us 4. Add 4 to the list of remainders.
Divide 156 by 16, which gives us 9.75. Multiply the remainder by 16, we get 12. Add 12 to the list of remainders. (Remember, in hexadecimal, 12 is C, so put C instead.) Our remainder list should be: 4C.
Since we know 9/16 is less than one, add 9 to the list of remainders without bothering to divide it. The remainder list is 4C9. Switch it around, and we get 9C4. 9C4 is hexadecimal for 2500.

Start time-6:46

452/16
(from my head, this won't make sense-45.2, 3(0); 48-45.2=2.8=28=1+12=27.75 (that took 2 minutes), C remainder.
27=1+11=F+B; plus another 2 minutes.
So…wait, no, not C, 4. (plus one more minute)
Ok. I think it's BF4, or F40B.
