Hexed items are things, which you can't get by normal game.

Hexing is when you do da haxs and get stuf 4 fwee.

Seriously- Hexing is going into the games save files and editing it using means of hexing. Some items are only obtainable through hexing.
Not many people will help you hex or give you hexed items 1. It's shunned upon on the forum and 2 I'd take an infinilots of explaining

HEX'ing is going into the game files and editing one of the .dat (or other) world or player file to get items or stuff that you don't want to get legitimately.
